The following assessment will identify any areas in which the team is failing to achieve results and areas of concern. On a scale of one (low evidence) to five (high evidence), select a value from the dropdown menu of your experience of each of the themes listed. Your answers should relate to the team you work in the majority of the time.

Remember: there are no right or wrong answers and honesty is the best policy!

Lack of trust between unit members or between members and the manager.
Decisions made that unit members do not understand or agree with.
The feeling that good work is not recognised or rewarded.
Lack of encouragement for unit members to work together in a better team effort.
